Solved now, at http://www.pmwiki.org/wiki/Test/EditForm2 .

With the five buttons at the bottom, the disparity in horizontal
widths of the buttons doesn't bother me as much as it once did.
However, I'm not happy with the labels "Save minor" or "Save and edit"
on the buttons.  Any suggestions?
