This sounds as if mod_rewrite is active (good), but none of the rules 
matched (surprising).

Please give the URLs that you use. The mapping from URLs to file names 
is nontrivial in Apache, and there might be some other effect.
Plus, preferrably, the associated access log, error log, and rewrite log.
