[pmwiki-users] page names with .shtml extension

Actually, what I came up with was a version of the last line of the
.htaccess file we use for clean urls:

# Send requests ending in .shtml to index.php.
RewriteRule ^([A-Z0-9\xa0-\xff].*)\.shtml$           index.php?n=$1  [QSA,L]

That does it, but it's still not elegant enough. I'd like to combine that
line with the last one, but so far it works. I'll add this version to the
documentation.
